Form Name and NumberDescription

Affidavit (Template) 

This form is a document sworn by you where you include the relevant facts to reply to the case against you. 

Civil Bill Possession  Form 2R   

This form is used to initiate a possession claim in the Circuit Court.

Endorsement of Service - Registered Post Form 1B   

This form is used to confirm that a document has been sent (served) to the other party via registered post. It must be completed by the person who sent the document.

Endorsement of Service - Personal Service Form 1C

This form is used to confirm that a document has been handed to the other party (served via personal service). It must be completed by the person who served the document.

Entry of Appearance Form 5 

This form is used for the Respondent (Defendant) to acknowledge the claim. It outlines whether the Respondent is representing themselves or is being represented by a solicitor.

Notice of Motion    

This form is used to apply for an order from the court on notice to another party in the case.

Certificate of non-appearance

This form is used by the Claimant (Plaintiff) to advise that no appearance has been entered by the Respondent(s) / Defendant(s).

Execution Order Form 23   

This form is used by the Claimant (Plaintiff) to apply for an execution order.
